The Healthy Canadians and Communities Fund (HCCF) encourages all sectors of society to participate in supporting healthy living. Our projects bring together partners to achieve better health outcomes for Canadians. The Healthy Canadians and Communities Fund (HCCF) was previously the Multi-sectoral Partnerships to Promote Healthy Living and Prevent Chronic Disease program (MSP).
We aim to improve health by supporting interventions that prevent chronic disease. These include diabetes, cardiovascular disease and cancer.
This interactive map is a visualization of funded interventions across Canada that support Canadians who face health inequalities and are at greater risk of developing chronic disease.
